1. แบบฟอร์มคำร้องขอสูติบัตรไทย
   (ดาวน์โหลดจากเว็บไซต์ของสถานเอกอัครราชทูต/สถานกงสุลไทยที่รับผิดชอบพื้นที่)
2. บัตรประชาชนไทย (ตัวจริงและสำเนา)
3. ทะเบียนบ้านไทย (ตัวจริง)
4. ใบสมรส (ถ้ามี)
5. หนังสือเดินทาง (พาสปอร์ต) ของบิดาหรือมารดาชาวไทย

เอกสารของลูก

1. สูติบัตรต่างประเทศ (Foreign Birth Certificate)
   • ต้องผ่านการรับรอง (Legalization / Authentication) จากหน่วยงานรัฐในประเทศที่ออกเอกสาร
   • บางประเทศต้องแปลไทยและรับรองโดยสถานทูตไทย
2. ใบรับรองจากโรงพยาบาลที่เด็กเกิด
   (รายละเอียดต้องมี เช่น ชื่อเด็ก วันเวลาเกิด น้ำหนักแรกเกิด ชื่อโรงพยาบาล ฯลฯ)

เอกสารของบิดาหรือมารดาที่เป็นชาวต่างชาติ (ถ้ามี)

• หนังสือเดินทาง (พาสปอร์ต) ของฝ่ายที่เป็นชาวต่างชาติ (ตัวจริงและสำเนา)

• สูติบัตรไทยสามารถขอได้ที่สถานทูตไทยในประเทศนั้นๆได้เลย
• ไม่มีจำกัดอายุ แต่ต้องพิสูจน์ได้ว่าเป็นลูกของคนไทยจริง

Documents Required From the Thai Parent

• Application form for a Thai Birth Certificate
  (Download from the website of the relevant Thai Embassy or Consulate)
• Thai National ID Card (original and a photocopy)
• Thai House Registration (Tabien Baan) – original
• Marriage Certificate (if applicable)
• Passport of the Thai parent (original and a photocopy)

Documents Required for the Child

• Foreign Birth Certificate
  • Must be legalized/authenticated by the issuing government authority
  • Some countries may require a Thai translation and further legalization by the Thai Embassy/Consulate
• Hospital Birth Record
  (Must include details such as the child’s full name, date and time of birth, birth weight, name of hospital, etc.)

Documents Required From the Non-Thai Parent (if applicable)

• Passport of the foreign parent (original and a photocopy)

Additional Notes

• There is no age limit for applying for a Thai birth certificate, but the parent(s) must be able to prove that the child is indeed the biological child of the Thai parent.

• A Thai birth certificate can be issued at the Thai Embassy or Consulate in the country where the child was born.
